If you have a question concerning your statement please call 259-3110.
Our clinical staff has been trained to answer most questions. They will consult with your doctor as necessary. It is very difficult for your doctor to interrupt the care of another patient to take phone calls. If necessary, the doctor will return your call as time permits later in the day.
Medical insurance is a separate contract and private arrangement between you and your insurance company. It is the patient’s responsibility to understand the benefits and limitations of their insurance plan.
We are a participating provider with Medicare and we have contracts with other major insurance carriers. Insurance issues should be addressed with the receptionist prior to your appointment. Patients must provide a copy of their insurance card upon request.
Our policy is payment of uncovered balances and copays at the time of service. If there is a financial question, please discuss it with the business office prior to your visit.
We understand the importance of keeping your health information private. A copy of our current privacy notice is available upon request. When requesting copies of clinical records please allow five days for processing.
Plan ahead when calling for a prescription refill. Up to 72 hours may be necessary for your doctor to review your chart and approve your medication.
Fee for missed Appointments:
We ask that you notify the office well in advance if you are unable to keep a scheduled appointment. We would like the option of offering that appointment to another patient who needs to see the doctor.
Please note that it is our office policy to assess a $25.00 fee for each missed appointment. This fee can not be billed to your insurance carrier.
